What you will do:

Configure, build, and maintain the Epic Health Planet module to support population health initiatives.
Collaborate with clinical and operational leaders to design workflows that support care management, preventive care, and chronic disease monitoring.
Analyze population health data and metrics to support the design and implementation of targeted care strategies.
Develop dashboards and reports to support performance improvement and monitor key quality indicators.
Assist in upgrades, testing, and change management for Health Planet and related modules.
Maintain up-to-date knowledge of Epic upgrades, functionality, and best practices in population health.
