Job Description
Duties:
- Provide social work services to patients and their families in a medical setting
- Conduct assessments to determine the psychosocial needs of patients
- Develop and implement care plans based on patient needs and goals
- Collaborate with healthcare professionals to coordinate and facilitate patient care
- Assist patients and families in accessing community resources and support services
- Provide counseling and emotional support to patients and families
- Advocate for patients' rights and ensure they receive appropriate care
- Document patient interactions and maintain accurate records
Experience:
- Master's degree in Social Work or related field
- Previous experience working as a Medical Social Worker preferred
- Knowledge of medical coding, research, and documentation practices
- Familiarity with pediatric care, nursing homes, assisted living facilities, special education, and developmental disabilities
